Jack H. Jacobus passed away, Thursday evening, December 14, 2017 in Rock Hill, SC. A graveside service will be held at 11:00 a.m., Saturday, December 16, 2017 in Polk Memorial Gardens, Columbus, NC with Dr. Chris Osborn officiating. Military rites will be conducted by the Polk County Veterans Memorial Honor Guard.
